  1. I purchased a secondhand shaft not knowing the Titleist adapter was different from the one that I currently have on my 816 H1 Hybrid.

Could someone please let me know what the adapter, on the right of the image, is meant to be for?

The one on the right looks like a knock-off of the fairway wood adaptor.
  3. If it's indeed a hybrid, it'd be from a 910, that's the only Titleist hybrid with a different surefit hosel than the rest.

Even the 910 fairways had the longer adapter. The other thing is the 913 and beyond for fairways and hybrids are 1/2* flatter. I think the spacing on the 910s are 3/4* for lie angles vs 1* for 913 and up.

Possibly from the 910H model as those were a bit longer.

Hi, I think Paul C gave you the correct answer.
If I remembered correctly, the only Titleist adaptors that does not fit to new head’s model are the hybrid 910 and anteriors. All the adaptors for drivers and wood of 910 fit to tsr.
