This winter I was fortunate enough to receive a sleeve of the prototype Titleist balls.  Recently I was finally able to get out and play thanks to some unseasonably warm weather.  This is the first time I've received something from Titleist and I was excited to be able to play the new balls, as well as give my feedback on their performance.  Unfortunately, I cannot find the insert that came in the sleeve with directions for completing the survey.  I was hoping that one of the Team Titleist members or someone from the Team Titleist staff could help me out with this.  Thanks in advance!
